Figtree <OHS> System Features Incident Management Provide an easy way to log an incident from either a mobile application or a simple web based interface for an employee at any level. Initial incident notification may only require users to record basic information which then initiates Figtree’s workflow engines to notify the correct personnel for review. The review stage can include coding the incident so it can be utilised in trend analysis & if an injury occurred map against AS 1885.1 for injury recording purposes & compliance. Injury management teams can then register Medical certificates, Medical Treatments & Return to Work Plans against the Injury. The ability to calculate Lost Time Injury Frequency (LTIFR) & other related frequency rates for accurate benchmarking & safety performance by not only the overall business but also by business unit also exists.
The incident management process facilitates Investigations to identify the root cause & other causal factors leading to the incident. This is done through a series of linked coded fields & free text.The coded fields ensure consistency throughout the investigation by providing a standard set of prompts. Once the causal factors are identified the Hazard and Risk management process is initiated. Once the causal factors are identified the Hazard and Risk management process is initiated. Our Hazard & Risk Management framework is designed around ISO 31000:2009 to ensure a standardised ERM approach to managing not only OHS risks but other risk types such as Financial, Project & Operational risks. As with incident reporting, Hazards can be notified by mobile application or a simple web based interface which then triggers Figtree’s workflow engines to notify the correct personnel for review. Incidents and hazards can be linked to the Risk Assessment process in order to prompt a review of the causes of repeat or serious incidents. Once hazards are identified the Risks can be assessed using an interactive risk matrix. By default, Figtree uses a (5 x 5) matrix of Likelihood x Consequences. However the system can be configured to suit each client’s business needs. Risk Analysis techniques such as ‘Bow Tie’ and ‘Cause and Effect’ analyses together with a range of output formats including ‘Fault Tree’ and ‘Fish Bone’ diagrams can be accommodated, if required.
Once a risk has been appropriately assessed, Risk Treatments (Controls) can be decided on by using the Hierarchy of Controls. Actions are then delegated to persons responsible either as a one off or as a re-occurring process for ongoing mitigation of the risk. Standard review processes are implemented to ensure Risks are reviewed & controls monitored. Managing contractors can be a difficult process but this burden can be reduced with the use of FigOHS software. Contractors’ details are logged into the system either by the business or contractors themselves, using a dedicated login process. Tracking insurance expiry & updating approved contractor register then becomes automated with reminders & escalations. Images of documents can be stored in the system for future reference. Individual Employees and Sub-contractors are linked to the Contractor where General Construction Induction Cards & Licences can be lodged & tracked along with any contact information. To ensure individuals understand specific Business & Safety requirements, an induction questionnaire is required to be completed to a set standard.
As with Figtree’s general methodology, these induction questions can easily be tailored to the client’s specific requirements and compliance standards/conditions set. Only Contractors who have successfully completed the induction appear in the Contractor Register, minimising the overall exposure to your business. Tasks & Job Safety Analysis reviews can also be logged against the contractor which is where the Tasks Management process is initiated. All Tasks undertaken by a business can be registered within FigOHS which leads to any associated Hazards & Risks being identified and assessed. Space Entry Working at Heights and Hot Work are managed by a permit system, tailored to the requirements of your business. To ensure Tasks are appropriately assessed prior to being undertaken, a Job Safety Analysis (JSA) can be attached to the Task. The JSA allows individual aspects of the Task to be assessed & Controls nominated to ensure a holistic view of the Task & identification of the associated Risks are identified. Each JSA & Permit can easily be printed off in a PDF format & taken to the site where the Task is undertaken. When the Task is completed the hard copy JSA & Permit can be loaded back into Figtree for validation. Figtree’s robust built-in Audit/Questionnaire facility allows your business to tailor audits to the needs of the business. Audits can be distributed to multiple users simultaneously with either a selection of audit sections or the entire audit to be completed. Audit results are accumulated into a single record, making reviewing & benchmarking a simple process. Each audit is scored & workflows can be triggered based on the result. The audits are version controlled and can be used for general surveys, housekeeping inspections through to full system audits. For example Figtree’s Audit/Questionnaire function can be used for Self-Assessments, OH&S Audits, regulatory audits, record handling (Complaints / Compliments). All fields, sections & modules within the database that the user has access to can be reported on in various formats, outputs can be ASCII, CSV, XML etc & different reports / views can be set for internal & external users.
